Planning for a Healthier North (PFAHN) is an alliance of over thirty health care providers from all health care sectors in Melbourne’s northern suburbs (population ~900,000+) . PFAHN was established to enable the alliance to work together at a planning and strategic level, to ensure that people in the north receive services through integrated, area based, interagency partnerships.
The consortium of over thirty providers includes all seven local government councils, four major acute hospitals, community health services and Divisions of General Practice, in addition to other providers including Royal District Nursing Services, Aboriginal and Torres Strait Islander services, Mental Health and Primary Care Partnerships. more….
